The Forms
The forms can be easily downloaded and typing in the information onto the blank template in .doc or .rtf format makes the form much easier to read. There is also no limit to the amount of text you can use as the boxes expand to accommodate it. The boxes on the blank forms are not very large and fill up very quickly when handwritten.
The following are the forms required. By clicking on the highlighted part of the text the form should appear on your screen provided you have a word processing package that will handle the format.
- Pre-Appraisal Forms - This pack contains Forms 1,2 & 3. These need to be completed and sent to your designated appraiser 2 weeks before your appraisal We have kept the guidance on form 3 as from last year. This was intended as a guide to the information required rather than a list to be ticked
- Post-Appraisal Forms - These are completed during and after your appraisal by your appraiser and form the appraisal report. More information on this on ‘The Process’
- Evaluation Form - This is your opportunity to provide feedback on your appraisal. The CPD tutor will handle these and feed the information back to the appraisers
- Non-Principal Claim Form - A form to claim you appraisal fee if you are working as a freelance locum with no regular attachment to a practice.
