The following describes the process which although unwieldy seems to work reasonably efficiently.
The GP Appraisal Co-ordinator will allocate an appraiser and write to the appraisee with contact details
Make contact with your appraiser about a month before the appraisal is due to be carried out to confirm a time, date and location
Complete forms 1,2 & 3 and submit along with any data to your appraiser 2 weeks before the appraisal date. These must be in e.format or (at the very least) typed. Handwritten documents are not longer acceptable (ref section 1 RCGP guidance). Alternatively, complete the paperwork online at www.appraisals.nhs.uk
Please ensure that the documentation includes the Form 4 from your previous appraisal
Have the appraisal on the agreed date
Appraiser and appraisee to complete and sign off Form 4. It is sensible to make two copies of this form. One to keep and one for the appraiser who will send this to Jo Smekens at the appraisal administration office at the postgraduate centre
Appraisee to complete PDP grid in form 4 with help from appraiser
Appraisee to submit evaluation form and PDP grid to CPD Tutor at the Postgraduate Centre
The new General Medical Practice Contract (from April 2004) global sum includes an element for continuing professional development and appraisal. No additional fees (e.g. for locum replacement costs) may therefore be claimed by GPs who are being appraised and who are included in a GMS or PMS contract with Milton Keynes PCT.
For sessional doctors not regularly employed by practices the PCT have agreed to pay a sum towards the cost. The rate for 2010-11 has been set at £300. Please submit your claim along with the rest of the documentation to the GP Appraisal Co-ordinator at the Postgraduate Centre. Please note that the claim form for 2010-11 must be received by the end of May 2011 at the latest. A copy of the claim form can be downloaded from “The Forms” page on this site.
